ATHLETIC FORMS
An MHSAA Physical form must be on file before an athlete is allowed to participate in athletics. A physical for the current school year must have been completed on or after April 15 of the previous school year.
Every athlete participating on a sports team at Centreville must read the Athletic Code and sign the Athletic Pledge. Also included in the packet is an Emergency/Insurance form that must all be completed and signed by the athlete and guardian. Athletic forms should be returned to the coach of the respective team.
