Registration Requirements

To register your child / children as a new student to our district--including Schools of Choice--the following items will be needed for registration. 

  • Certified Birth Certificate (original). We will NOT keep this. 
  • Verification of Residency: Copy of mortgage / rental / lease agreement, property tax bill, gas bill, cable bill. This must include your name & address on it.
  • Our online registration form completed. See link below.
  • Court documentation / guardianship papers. If applicable. 
  • Free / Reduced Lunch form. Please fill this out even if you don't think you would qualify.

A family's checklist: 

  • Verify Centreville Public Schools is your home district. If not, refer below to the Schools of Choice information. 
  • Submit your child(ren)'s completed application via the PowerSchool link provided below. 
  • Required documentation can be uploaded to the online application or submitted to the office (uploads preferred).

Schools of Choice Forms

Schools of Choice Information

The nine school districts in St. Joseph County, with the support of the ISD, offer a Schools of Choice Program that allows families a choice to attend any public school within St. Joseph County ISD.

Applications for St. Joseph County Schools of Choice Program have been designed to guarantee all applicants a fair and equitable opportunity for choosing a school and are available from participating school districts during regular school hours.

A "Program Application" must be completed for each student (including siblings) wishing to participate in the program.

New student registration through PowerSchool enrollment is intended for New Students who live in the Centreville School District and wish to attend Centreville Schools. If you do not live within the Centreville School District and would like to apply to the Schools of Choice program, please scroll up to the "Schools of Choice Application" tab. There, you will find all necessary forms.

You will need to fill out and submit a Schools of Choice application for approval prior to completing a registration form.

Once the forms are completed, you can either bring them to the main office or scan them and send them to the designated email. Information listed below:
